Covering Yourself
All full-time Haselden team members regularly scheduled to work 30+ hours per week
Covering your Family Members:
Your spouse/domestic partner and/or children may be covered for benefits under the plan as long as:
- They meet the eligibility requirements as outlined in the Summary Plan Description (SPD)
- You are enrolled in the benefit
- You have proof of dependent status

As a new team member:
- You can make your benefit elections after you receive your first paycheck. You must make your elections prior to your benefits effective date
- Your benefit elections will go into effect the first of the month following 30 days of employment
As a current team member:
- You may only enroll in benefits or make changes to your current benefit offerings at open enrollment each year
Note: you may only make changes to your benefit elections outside of the above enrollment periods if you experience a qualifying life event.

A qualifying life event includes, but is not limited to:
- Marriage/divorce/legal separation
- Birth of a child (including adoption)
- Death of a spouse/child
- Loss of other coverage (e.g., child turns 26, loss of coverage through spouse’s plan)
- Open enrollment event (either at Haselden or your spouse’s company)
Please remember, if you experience one of these events and wish to make changes to your benefits, you must submit these changes via UltiPro within 30 days of the qualifying event. Otherwise, you will have to wait until the next Open Enrollment period. Supporting documentation may be required to make a change.

Open enrollment happens once each plan year. Haselden usually holds their Open Enrollment in late-October or early-November.
Haselden sends out numerous communications and reminders about Open Enrollment to our team members via email and home mailers.
