A deductible is the amount you must pay out-of-pocket each plan year for eligible services before the plan begins paying toward any additional covered services (except preventive care, which is paid 100% by the plan)
There are two different deductibles on your medical plan. An individual deductible for those enrolled in Team Member Only coverage and a family deductible for those enrolled in any Team Member + Dependent coverage.
Please note, the family deductible is meant to serve as a safety net for a family’s worst-case scenario. The most any one person in a family will pay for a deductible is the individual deductible.

By completing your annual wellness exam, Haselden will deposit $750 into your HSA account. An additional $250 will be deposited if your spouse also completes their annual exam.
