A deductible is the amount you must pay out-of-pocket each plan year for eligible services before the plan begins paying toward any additional covered services (except preventive care, which is paid 100% by the plan)

There are two different deductibles on your medical plan. An individual deductible for those enrolled in Team Member Only coverage and a family deductible for those enrolled in any Team Member + Dependent coverage.

Please note, the family deductible is meant to serve as a safety net for a family’s worst-case scenario. The most any one person in a family will pay for a deductible is the individual deductible.

By completing your annual wellness exam, Haselden will deposit $750 into your HSA account. An additional $250 will be deposited if your spouse also completes their annual exam.

By using untaxed dollars in a Health Savings Account (HSA) to pay for deductibles, copayments, coinsurance, and some other expenses, you may be able to lower your overall health care costs.

1) Text HSA to 343898 to be provided with a link to Fidelity and more information.

2) Click the link to head to the Fidelity website

3) To set up your HSA, select Get Started and log into fidelity using your 401k credentials. If you don’t have credentials, go to New User and register now.

4) Follow the steps to set up an HSA. Notify payroll@haselden.com of your desired contribution amount, if any. This can be changed at any time.
